Clinical Case Story: Perforated Appendix Leading to Exploratory Laparotomy
Today, a patient presented to our facility with lower abdominal pain, associated with tachycardia and fever. On examination, there was marked tenderness and guarding — strongly suggestive of peritonitis.
A provisional diagnosis of perforated appendix was made, and the patient was urgently taken for Exploratory Laparotomy.
Intraoperative Findings:
Perforated, gangrenous appendix Generalized peritonitis Purulent fluid in the abdominal cavity
An appendectomy was performed. The abdominal cavity was thoroughly irrigated with warm normal saline to reduce the bacterial load. A pelvic drain was placed for postoperative drainage, and due to contamination, the wound was left open for healing by secondary intention.

آج ایک مریض کو پیٹ کے نچلے حصے میں شدید درد کے ساتھ اسپتال لایا گیا۔ معائنے کے دوران ٹینڈرنیس اور گارڈنگ واضح طور پر موجود تھی، ساتھ ہی مریض کو تیز بخار اور دل کی دھڑکن میں اضافہ (ٹیکی کارڈیا) بھی تھا۔
تشخیص کی گئی کہ یہ کیس پرفوریٹڈ اپینڈکس کا ہو سکتا ہے، جس کے بعد مریض کو فوری طور پر ایکسپلوریٹری لیپروٹومی کے لیے لے جایا گیا۔
آپریشن کے دوران درج ذیل مشاہدات ہوئے:
اپینڈکس سیاہ، گل سڑ چکا اور پھٹا ہوا تھا پیٹ میں پیپ اور بدبو دار مادہ (جنرلائزڈ پیریٹونائٹس) ارد گرد انفیکشن پھیل چکا تھا
ہم نے اپینڈیکٹومی کی (پھٹا ہوا اپینڈکس نکالا)، پھر نارمل سیلائن سے پورے پیٹ کی صفائی کی تاکہ انفیکشن کم ہو۔
ایک ڈریں پیلوِس میں لگائی گئی تاکہ پیپ باہر آ سکے، اور زخم کو کھلا چھوڑ دیا گیا تاکہ وہ خود سے بھر سکے۔
سبق:
اگر اپینڈکس کا علاج بروقت نہ کیا جائے تو یہ خطرناک پیچیدگیوں کا سبب بن سکتا ہے۔ جلد تشخیص اور فوری سرجری ہی جان بچانے کا واحد راستہ ہے۔
